Focus Long Term Home Health Care Program (LTHHCP) at Otsego

The Long Term Health Care Program (LTHHCP) is a home care program particular to New York State. The LTHHCP was established by Senator Tarky Lombardi in 1977 as a viable alternative to institutional care. It is referred to as the Lombardi Program, the Nursing Home without Walls, or the Long Term Home Health Care Program.

The Lombardi Program provides and coordinates the delivery of comprehensive services to the chronically ill and disabled of any age in their place of residence. Their place of residence includes the patient’s home or the home of a relative.


Persons of any age who are chronically ill, disabled and/ or infirm and meets the following criteria are eligible:
  • Has a skilled need
  • Is an Ostego County resident
  • Is medically eligible for skilled or health related nursing home care
  • Has a payment source
  • Can be cared for safely in their residence

How to receive care

Referral may be made directly from a hospital, a physician, a nursing home, a family member, or from a person in the community. A Home Health Registered Nurse will then do an assessment in your home to determine eligibility for the program.
